Neuraxiom.com (the “Site”), an educational website operated by the Middle Tennessee School of Anesthesia (“MTSA” or “We” or “Us”), automatically collects and stores certain limited information about visitors to the Site.  This information includes visitors’ internet protocol (IP) addresses, the region or general geographical location from where you are accessing the internet, browser type, and operating system.  Information about how visitors use the Site is also collected, including a history of the pages you view, and time spent on individual pages. This information is used by us to aid our future design and development of our Site.  IP addresses may also be used by us to diagnose problems with the Site, to administer the Site, analyze usage trends, track visitor movements, and gather broad demographic information.  We may also record clicks of hyperlinks to third party websites accessed from our Site.

 

The Site also offers visitors the opportunity to provide personally identifiable information, such as your name, email address, and phone number, to allow us to provide you with information about certain program offerings. We will use this information to contact you about certain MTSA-affiliated programs, as well as related activities and offerings.  The Site also has functionality to allow users to post questions on the Site’s forum.  No health information that could potentially identify any individual should be provided using this functionality, however MTSA may use and publicly disclose any information submitted to the forum.  Additionally, any personal identifiable information submitted to the forum may be used to contact you, and provide you with information about certain MTSA-affiliated programs, as well as related activities and offerings.
